- Assess patient needs and plan care accordingly
- Monitor and evaluate care through ongoing assessments

- Document patient interactions and care modifications
Requirements:
- Active registration with the College of Nurses of Ontario
- RPN diploma from an accredited institution
- Valid First Aid or CPR Certification
- Experience in acute care nursing preferred
- Reliable vehicle for home visits required
