Provide patient-centered social work across Vancouver Island Health Authority sites, using assessment and care planning to address barriers to care. Travel between multiple sites may be required.
This regular full-time Social Worker supports a multidisciplinary health care team. The position provides assessment, intervention, consultation, and referrals when social, economic, emotional, or psychological circumstances affect care or function. A Master's degree in Social Work is preferred.
Key Responsibilities:
- Assess patients' social, economic, emotional, and psychological circumstances
- Develop care plans and provide patient-centered interventions
- Consult with patients, families, staff, and health care providers
- Refer patients to appropriate community and health services
- Respond to emergent situations and coordinate workload
Requirements:
- Eligibility for registration under the Social Workers Registration Act
- One year of recent related experience, or equivalent education
- Working knowledge of legislation and community resources
- Comprehensive counseling, crisis counseling, and suicide assessment skills
- Effective verbal, written, independent, and multidisciplinary communication
