You will deliver clinical and consultative social work services within a client- and family-centered care model, conducting assessments, providing counselling, and coordinating care across the continuum.
As part of Primary Care Networks, you’ll work with a multidisciplinary team to support patients and families, connecting them to appropriate resources and enabling access to timely care.
