Vancouver Coastal Health seeks a Mental Health Rehabilitation Worker for the Tertiary Mental Health - Assessment & Treatment team. Working within an interdisciplinary team, you provide day-to-day physical, emotional, and social supports for individuals with severe and persistent mental illnesses. You help inform individualized care plans, monitor progress, symptoms, and behavioural changes, and support community reintegration through functional skills, coping strategies, and independence.
Key Responsibilities:
- Provide comprehensive psychosocial rehabilitation services within a recovery framework
- Support activities of daily living, appointments, errands, and community activities
- Assist with life skills, meal planning, and meal preparation
- Observe client progress, symptoms, and behavioural changes
- Maintain therapeutic relationships and contribute to care evaluations
Requirements:
- Grade 12 and recognized mental health program certificate
- Equivalent combination of education, training, and experience considered
- Communicate effectively verbally and in writing
- Organize work and operate related equipment
- Physical ability to carry out position duties
