Apply clinical pharmacy expertise to direct patient care and drug therapy optimization at Two Rivers Family Health Team. This position supports multidisciplinary practice.
The Clinical Pharmacist provides primary care support through patient assessments, therapeutic drug monitoring, and evidence-based drug information. The position also supports chronic disease management, quality improvement, and team education. Employment includes 0.6 FTE maternity leave coverage with 0.4 FTE potential for permanent employment.
Key Responsibilities:
- Assess patients to identify, prevent, and resolve drug therapy problems
- Conduct therapeutic drug monitoring for individual patient care
- Provide unbiased drug information to the multidisciplinary team
- Participate in chronic disease management programs, including Memory Clinic
- Lead provider responsibilities in the Anticoagulation Clinic
Requirements:
- Ontario College of Pharmacists licence
- Minimum one year relevant clinical pharmacy experience in primary care
- Computer literacy; electronic medical records experience is an asset
- Strong organizational, interpersonal, and communication skills
- Ability to work independently and cooperatively within multidisciplinary teams
