Lead anticoagulation care while providing direct patient assessments and therapeutic monitoring at Two Rivers Family Health Team. The assignment includes flexible FTE potential.
As Clinical Pharmacist, you will optimize drug therapy regimens and deliver evidence-based information within a multidisciplinary team.
You will lead the Anticoagulation Clinic, participate in chronic disease programs, and contribute to quality improvement.
The position provides 0.6 FTE maternity leave coverage and 0.4 FTE potential for permanent employment.
Experience managing warfarin is an asset.
Key Responsibilities
- Lead provider activities for the Anticoagulation Clinic
- Monitor therapy and resolve individual drug therapy problems
- Support chronic disease management initiatives and Memory Clinic programs
- Contribute to ongoing Pharmacy program quality improvement initiatives
- Assist with pharmacy and allied health student teaching
Requirements
- Pharmacy licence with the Ontario College of Pharmacists
- At least one year relevant clinical pharmacy experience
- Anticoagulation or warfarin management experience is an asset
- Chronic pain and opioid management experience preferred
- Injection certification is an asset
