Key Responsibilities
- Deliver high-quality, patient-centered care in collaboration with our interdisciplinary team.
- Conduct comprehensive assessments, diagnoses, and treatments for a diverse patient population.
- Develop and implement personalized care plans, ensuring continuity of care.
- Engage in health promotion and disease prevention strategies to improve patient outcomes.
- Maintain accurate and timely documentation of patient interactions and care provided.
- Participate in quality improvement initiatives to enhance service delivery and patient satisfaction.

Requirements/Qualifications
- Current registration as a Nurse Practitioner with the College of Nurses of Ontario.
- Master’s degree in Nursing or equivalent education and experience.
- Proven clinical experience in a healthcare setting
- Excellent communication and interpersonal skills, with a compassionate approach to patient care.
- Strong organizational skills and the ability to work independently and collaboratively.
