Vaughan Community Health Centre seeks a Family Physician for permanent, full-time practice at 9401 Jane St. This clinician serves clients of all ages and families through health promotion, disease prevention, education, screening, referrals, and follow-up. Care is delivered in person and virtually for local and underserved communities.
Key Responsibilities:
- Provide primary care to individuals of all ages and families
- Perform histories, advanced assessments, diagnosis, and treatment planning
- Deliver clinic, community, home-visit, urgent, and on-call services
- Coordinate hospital and community healthcare during client transitions
- Maintain records and support quality improvement strategies
Requirements:
- Medical degree and recognized family medicine residency
- CCFP certification and Ontario registration in good standing
- Two to five years' primary care experience, including residency
- Proficiency with EMR systems and Ontario healthcare technology
- Experience providing culturally and linguistically responsive care
