You will provide education, clinical support, healthcare navigation, and follow-up while collaborating with providers, pharmacies, and reimbursement teams. Patient advocacy and trusted relationships guide the work.
Key Responsibilities:
- Deliver patient-centric case management within established program protocols
- Provide therapy education, adherence support, and ongoing follow-up
- Coordinate care with physicians, clinics, pharmacies, and internal teams
- Document interactions and monitor treatment progress accurately
- Report adverse events and product complaints according to requirements
Requirements:
- Diploma or degree in nursing
- Current unrestricted Alberta nursing registration in good standing
- Direct patient care, education, support, coordination, or case management experience
- Strong English communication and Microsoft Office proficiency
- Valid driver's license and reliable transportation
