Provide patient-centred nursing care on Unity Health Toronto's 4M Medicine unit, using cardiac assessment and telemetry monitoring skills. Support patients with heart and medical conditions.
As a Registered Practical Nurse, you will deliver holistic, evidence-based care for stable patients with predictable outcomes. The 34-bed unit includes a Cardiology sub-focus and patients requiring telemetry monitoring. You will collaborate with Registered Nurses and interprofessional colleagues while responding to changing patient conditions.
Key Responsibilities:
- Provide autonomous nursing care for stable patients
- Assess needs and develop individualized plans of care
- Monitor clinical data and adjust care plans
- Educate patients and families using effective strategies
- Coordinate safe transitions and communicate patient care activities
Requirements:
- Practical Nursing Diploma and Ontario registration
- Current BLS/CPR certificate
- Coronary Care I or equivalent required
- Recent acute inpatient medicine experience
- Cardiac, respiratory, and telemetry experience preferred
