Southlake Health is seeking a Nurse Coordinator to lead the Southlake@home transitions. You will coordinate care, liaise with patients, families and providers, and ensure safe hospital-to-home transitions.
Reporting to the Manager, Community Programs, you will support discharge planning, referrals and ongoing program evaluation in a fast-paced environment across hospital and community settings.
