Shoppers Drug Mart seeks a Registered Nurse for a full-time field case management position serving London and surrounding communities. You will coordinate patient and physician caseloads, support therapy progression, and provide education through home, clinic, hospital, telephone, and virtual interactions. The role works from home with approximately 50 to 75 percent travel.
Key Responsibilities:
- Coordinate clients from enrollment throughout therapy
- Assess disease status and monitor adherence and side effects
- Provide patient, care partner, and provider education
- Document care changes and maintain continuity
- Coordinate reimbursement, dispensing, delivery, and refill updates
Requirements:
- Current unrestricted Registered Nurse license
- Five or more years of direct clinical experience
- Recent Patient Support Program experience
- Fluency in written and spoken French and English
- Valid driver's license and reliable vehicle
