You will provide client-centered, community-based in-home services for eligible, high-risk seniors. The work supports daily living, health, safety, and independence according to service plans.
Key Responsibilities:
- Assist clients with bathing, dressing, eating, hygiene, and ambulation
- Document care, changes, and findings in the CSS EMR
- Coordinate transportation, appointments, medications, meals, and community resources

- Provide well-being checks, cleaning, errands, and social support
Requirements:
- Secondary school diploma or equivalent knowledge, skills, and experience
- Valid CPR certification and Class G driver's license
- Safe driving record and effective communication skills

- Ability to work independently, prioritize needs, and maintain confidentiality
