Key Responsibilities:
- Assess and engage new clients with health care team
- Document assessments and implement care plans
- Facilitate team meetings for case management

Requirements:
- Registration with BC College of Nurses and Midwives
- Two years of experience in primary care/mental health
- Certification in Point of Care HIV Testing
- Knowledge of trauma-informed care and harm reduction
- Ability to perform venipuncture and CPR procedures
