In this role, you will work within an interdisciplinary care team, focusing on providing assessment and stabilization for individuals dealing with schizophrenia and complex mental health issues. Your clinical social work acumen will guide you in performing psychosocial assessments and delivering individual, group, and couple therapy. Collaborating with community agencies will enhance discharge planning and coordination, ensuring a supportive patient experience.
Key Responsibilities:
- Conduct psychosocial assessments and develop treatment plans
- Facilitate individual, group, and family therapies
- Liaise with community social agencies for comprehensive care
- Collaborate as an integral part of the interdisciplinary team
- Coordinate patient discharge planning and support
Requirements:
- MSW and registration with OCSWSSW
- 3-5 years psychiatric experience in mental health
- Knowledge of Psychosocial Rehabilitation and Recovery
- Competence in therapeutic modalities including CBT and DBT
- Excellent communication skills and cultural competency
