Ontario Health atHome is seeking a Temporary Float Hospital Care Coordinator to assess patient care needs, determine eligibility, and connect patients with community services across hospital sites in Ontario Health atHome Toronto Central.
You will lead patient-centered care, coordinate with families and health care teams, and support care plans while rotating shifts of eleven hours from 09:00 to 20:00, biweekly three and four days, including every second weekend.
