This temporary full-time Care Coordinator position serves Ontario Health atHome's Community program through the Scarborough branch. You will assess eligibility, develop care plans, connect patients with providers, and monitor service delivery. The work involves patients, families, caregivers, and health care practitioners.
Key Responsibilities:
- Assess care needs with patients and families

Requirements:

- At least two years' recent community health experience
