Nova Scotia Health seeks a Health Care Social Worker for the OPCZ Social Work HCH department. You will contribute to interdisciplinary care across the primary to quaternary continuum. The work focuses on psychosocial, emotional, and spiritual wellbeing through engagement, planning, intervention, and evaluation.
Key Responsibilities:
- Provide collaborative, holistic care within an interdisciplinary health care team
- Engage patients and clients through social work practice
- Assess social determinants of health and individual needs
- Plan interventions using strengths and existing coping strategies
- Evaluate care while supporting problem-solving skills and personal resources
Requirements:
- Bachelor's degree in Social Work from a recognized university
- Current registration with the Nova Scotia College of Social Workers
- Two years of post-BSW related clinical experience
- Knowledge of the Diagnostic and Statistical Manual of Mental Disorders
- Client- and family-centered approach with group facilitation experience
