Key Responsibilities:
- Provide education and support for diabetes self-management
- Implement and assess client care plans regularly
- Collaborate with dietitians for holistic health evaluations
- Execute medication reviews and recommendations
- Contribute to organizational quality improvements and reports
Requirements:
- Bachelor's degree in Nursing
- 1-3 years of clinical/community experience
- Good standing registration with the CNO
- Diabetes education experience is beneficial
- Valid Class G driver's license with clear check
