The Ma Mawi Wi Chi Itata Centre seeks Care Planners for full-time term positions through March 2028. Working with the Wi Che Win team, you will provide holistic support services, conduct home visits, and support individualized plans. The work helps participants move from homelessness toward housing.
Key Responsibilities:
- Provide individual support services and conduct participant home visits
- Assist participants in acquiring and maintaining homes
- Support access to emergency housing when needed
- Help participants develop tenant and neighbour skills
- Respond immediately to crises and assess participant needs
Requirements:
- Youth Care Diploma or acceptable equivalent with related experience
- Care planning and healing plan development experience
- Working knowledge of crisis intervention strategies
- Experience building relationships with Indigenous participants
- Valid Manitoba Driver's Licence and required background checks
