The Patient Navigator provides leadership across acute and post-acute care areas. This role coordinates bed management, repatriations, and patient flow challenges with stakeholders. It also supports process improvement, quality, safety, and access through facilitation, communication, and change leadership.
Key Responsibilities:
- Facilitate bed management and organizational flow with stakeholders
- Coordinate repatriations to appropriate JBH programs and services
- Attend rounds to anticipate bed demand and supply
- Process escalations for patients requiring Alternative Level of Care
- Coach staff on processes, decisions, policies, and procedures
Requirements:
- Healthcare professional in good standing in Ontario
- Bachelor's degree preferred in healthcare or related discipline
- Minimum five years in acute care or emergency settings
- Knowledge of patient flow, legislation, and community resources
- Strong communication, problem-solving, and multidisciplinary collaboration skills
