Deliver patient-centered mental health care at Joseph Brant Hospital using therapeutic interventions, clinical judgment, and community discharge planning. This regular full-time position serves Burlington.
As a Case Manager and Community Mental Health Clinician, you will support safe, timely, and effective discharge to the community. The position requires clinical experience in Mental Health and Co-Occurring Addictions, strong engagement skills, and family support expertise. Days and evenings are scheduled, with flexible hours including weekends available.
Key Responsibilities:
- Provide patient-centered care within Mental Health Ambulatory Services
- Support safe, timely, effective discharge to the community
- Apply therapeutic interventions with challenging family dynamics
- Provide family support and education individually and in groups
- Document care and maintain patient confidentiality
Requirements:
- Allied professional discipline, including MSW, OT, or Recreation Therapy
- Membership in good standing with the appropriate regulatory body
- Five to seven years recent related clinical experience required
- Group experience and Intensive Case Management familiarity
- Valid driver's license and access to a vehicle
