Stonehenge is seeking an Addiction Support Coordinator to provide direct services for individuals with addiction and co-occurring conditions.
Most work occurs in clients’ homes; travel across the Waterloo Wellington LHIN is required, including rural areas.
Occasional evenings and weekends may be required.
Responsibilities include counselling, coordinating access to primary care and supports, risk assessment, referrals, and discharge planning, with collaboration across community agencies.
