This permanent full-time Pharmacist serves within OT Clinical Pharmacy at Halton Healthcare. The position evaluates medication use, supports formulary management, interprets clinical literature, and provides medication information. You will collaborate with physicians, pharmacists, nurses, and healthcare leaders to improve patient-centered medication therapy.
Key Responsibilities:
- Develop and improve the organization's Drug Use Evaluation Program
- Evaluate prescribing practices, medication therapy, and clinical outcomes
- Provide evidence-based medication information to healthcare teams
- Support formulary management and medication-use policy development
- Present recommendations and outcomes to committees and stakeholders
Requirements:
- BScPhm, PharmD, or equivalent qualification
- Ontario College of Pharmacists Part A registration in good standing
- Minimum five years of relevant hospital clinical experience
- Strong analytical, critical-thinking, and problem-solving abilities
- Excellent written, verbal, presentation, and stakeholder engagement skills
