Fraser Health is hiring a relief full time Registered Nurse for Community South Fraser. You will assess adults and geriatric clients, coordinate care, and provide clinical interventions across community, clinic, and home settings. The work includes care planning, health counseling, referrals, and collaboration with interprofessional teams.
Key Responsibilities:
- Establish therapeutic relationships respecting client autonomy, dignity, and privacy
- Complete individualized assessments and coordinate services for complex chronic conditions
- Develop shared care plans with clients, families, and providers
- Provide direct care, education, and health counseling
- Maintain clinical records and client statistics according to policies
Requirements:
- Current practicing registration with BCCNM as a Registered Nurse
- One year recent related clinical experience with complex patients
- Recent care and discharge planning experience, or equivalent preparation
- Valid British Columbia driver's license and personal vehicle access
- Ability to communicate effectively verbally and in writing
