This MSW position delivers clinical social work services within an integrated interdisciplinary team.
You will assess clients and families experiencing acute episodes of serious and persistent mental illness, develop care plans, and support continuity after hospitalization.
The position also involves consultation, referrals, advocacy, and community coordination.
Key Responsibilities
