Overview
Clinical Pharmacist – Langley Memorial Hospital – Part‐time (18‐month contract) with Fraser Health Authority.
This role utilizes clinical pharmacy expertise to advance patient medication therapy within a collaborative hospital environment.
Salary
Hourly rate: CAD $51.37 – $64.19 (part‐time hours). Competitive benefits package is available.
Responsibilities
- Individualized patient medication therapy and counselling.
- Collaboration with physicians, nurses and other health providers to develop, implement and monitor therapeutic plans.
- Development of drug therapy guidelines for disease conditions and preparation of professional journal submissions.
- Participation in medication use evaluations, quality improvement initiatives and clinical drug trials.
- Mentorship and supervision of pharmacy students, residents and new staff.
- Prescription validation and dispensing accuracy, documentation of safety concerns and resolution of discrepancies.
Qualifications
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Completion of an accredited Hospital Pharmacy Residency Program (or equivalent combination of education, training and experience).
- Broad knowledge of evidence‐based clinical pharmacy practice and medication use evaluation.
- Ability to communicate effectively, establish working relationships and operate related equipment.
- Physical ability to carry out the duties of the position.
