Fraser Health seeks a Licensed Practical Nurse, Full Scope, for Home Health in Port Moody, B.C. This community-based position manages assigned caseloads and supports clients with complex chronic conditions. The work includes direct care, health counseling, transitions, and collaboration with interprofessional providers.
Key Responsibilities:
- Establish therapeutic relationships respecting client autonomy and privacy
- Assess clients and develop individualized interprofessional care plans
- Provide direct care, counseling, education, and self-management support
- Coordinate transitions across primary, community, and acute care
- Maintain clinical records and participate in quality improvement
Requirements:
- Current practicing BCCNM registration as a Licensed Practical Nurse
- One year recent related clinical experience
- Experience with complex geriatric or adult chronic conditions
- Valid British Columbia driver's license and personal vehicle access
- Ability to communicate effectively verbally and in writing
