Fraser Health is seeking a Licensed Practical Nurse (LPN) Outreach to join the multidisciplinary mental health services team in Burnaby. You will deliver assessments, plans, and nursing care to clients in their homes and in the community, following BCCNM standards and Fraser Health policies.
Reporting to the Program Coordinator, you will work with RNs and RPNs, coordinate care, document outcomes, and participate in interdisciplinary planning.
