Key Responsibilities:
- Coordinate Community Support Workers delivering services in the field
- Supervise and coach healthcare teams across assigned communities
- Develop and maintain effective Community Health Services Care Plans
- Monitor employee performance and applicable healthcare practices
- Provide workplace health and safety guidance
Requirements:
- Registered Nurse or Registered Practical Nurse degree or diploma
- Three to five years of related experience or equivalent
- Registration with the College of Nurses of Ontario
- Knowledge of case management and nursing processes
- Current CPR Certification and required background checks
