You will follow policies, procedures, work routines, and nursing care plans while supporting patients' intellectual and spiritual needs. Key Responsibilities:
- Provide direct personal support and care to patients
- Assist patients with ambulation, feeding, bathing, and grooming
- Address physical, intellectual, emotional, social, and spiritual needs
- Follow policies, procedures, and established work routines
- Complete care documentation using care planning systems
Requirements:
- Completed PSW certification meeting appropriate position standards
- Experience in long term care or community home care preferred
- Exposure to dementia and palliative care preferred
- Ability to read and write English and understand instructions
- Reliable vehicle required for community home care
