Durham Community Health Centre seeks a Registered Dietitian—Diabetes Educator for its Diabetes Education Program. Working within an interdisciplinary team, you will provide education, consultation, and liaison to clients, caregivers, families, and service providers. The position supports diabetes self-management and comprehensive continuous care.
Key Responsibilities:
- Perform nutrition assessments and counselling for individuals
- Co-facilitate group education sessions
- Advocate for clients and families on diabetes self-management
- Participate in case conferences and provide consultation
- Develop, monitor, and evaluate Diabetes Education Program protocols
Requirements:
- Undergraduate food and nutrition degree with accredited dietetic internship
- Registration with the College of Dietitians of Ontario
- Three to five years' current diabetes education practice
- Program development, implementation, monitoring, and evaluation experience
- Valid Ontario driver's licence
