Durham Community Health Centre (DCHC) is seeking compassionate Case Workers to join the HART Hub 24/7 stabilization team in the Whitby and Durham Region area. The role focuses on building relationships, supporting residents, and coordinating housing and services.
You will work Saturday and Sunday, 12 PM–12 AM, on a part-time contract with HOOPP participation, contributing to individualized stabilization plans and client-centred care.
