Durham Community Health Centre (DCHC) seeks a Nurse Practitioner to join the Youth Wellness Hub, providing youth-centered primary care and wellness supports. Services occur virtually, on-site, and in the community, following the YWHO model, with emphasis on trauma-responsive and culturally safe care.
The NP collaborates with inter-professional Hub staff to ensure integrated care, continuity, and effective navigation of health and social services for youth, including mental health and substance use.
