Support vulnerable individuals and families as a Housing Social Worker with the City of Windsor, providing intensive case management and counselling. This regular or temporary full-time position requires shift work.
As a Social Worker, Housing, you will serve clients experiencing homelessness or housing vulnerability. Your work includes case planning, crisis intervention, bio-psychosocial assessment, community referrals, and risk assessment. You will collaborate with health, justice, housing, and social service partners.
Key Responsibilities:
- Provide intensive on-site and in-home case management
- Create and regularly update client case plans
- Deliver crisis intervention and clinical counselling
- Assess needs and make community resource referrals
- Coordinate interdisciplinary case conferences for complex needs
Requirements:
- Four-year university degree in Social Work
- Over one year of relevant work experience
- Registration with Ontario's social work college
- Ability to travel to offsite locations
- Microsoft Suite experience in computerized office environments
