CBI Home Health is seeking an After Hours Integrated Care Lead for the Newmarket/Bradford region on a 12-month contract.
You will oversee after-hours integrated care, coordinate hospital discharge planning and ensure patient-centered transitions home.
You will work rotating 12pm-8pm shifts, collaborate with hospital teams and families, and develop care plans that support safe discharges and sustainable home care.
Ontario-registered health professional with 3–5 years supervision preferred.
