The position supports Nurses throughout Norfolk County and works from the St. Catharines office approximately one day weekly. Work includes Monday through Friday hours and rotational Branch Manager On-Call evenings and weekends. The role supports high-quality patient care, healthcare team mentoring, and leadership across community settings.
Key Responsibilities:
- Coordinate and supervise Nurses delivering community health services
- Coach Nurses working in the field
- Develop effective Community Health Services care plans
- Monitor employee performance and nursing practices
- Provide workplace health and safety guidance
Requirements:
- Registered Nurse degree and CNO registration
- Three to five years of related experience
- Knowledge of case management and nursing processes
- Computer skills including scheduling systems and word processing
- Vehicle, regular travel, and clear vulnerable sector check
