Baycrest Hospital seeks a Registered Practical Nurse with complex-care expertise for temporary part-time day shifts and alternate weekends.
This Registered Practical Nurse supports older adults in Complex Continuing Care Unit 5W. The position provides post-acute inpatient care for patients with multiple chronic health conditions. It emphasizes client-centred nursing within an interdisciplinary team, including medications, treatments, and individualized care planning.
Key Responsibilities:
- Plan nursing interventions, medications, and treatments within scope
- Deliver and evaluate nursing interventions for complex medical needs
- Provide client-centred personal care, activation, recreation, and spiritual support
- Collaborate with Registered Nurses on individualized care plans
- Revise individualized care plans with the interdisciplinary team
Requirements:
- Current good-standing registration with the College of Nurses of Ontario
- Current Basic Cardiac Life Support Level C and AED certificate
- Geriatric complex medical care experience preferred
- Psychogeriatric assessment, intervention, or responsive behaviours experience preferred
- Excellent interpersonal, communication, and problem-solving skills
