As an experienced LPN at AgeCare, you'll report to the Director of Care while being part of an inter-professional team. You will conduct accurate clinical assessments, facilitate care plans, and administer resident treatments in compliance with regulatory standards. A strong focus on teamwork and person-centered care is essential to ensure comprehensive support for our residents and their families.
Key Responsibilities:
- Conduct clinical assessments and maintain accurate care plans
- Implement and evaluate effectiveness of care/service plans
- Participate in scheduling and conducting care conferences
- Assist with resident move ins, outs, and transfers
- Provide direct care and administer medications as needed
Requirements:
- Practical Nurse Diploma and current registration
- Minimum 3 years' experience in seniors' health
- Current CPR certification required
- Proficiency in Microsoft Office applications
- Demonstrated leadership and mentorship abilities
