You will collaborate within an inter-professional team, maintain current assessments and plans, and communicate changes. Care follows provincial standards, Resident Care requirements, and AgeCare policies.
Key Responsibilities:
- Collaborate within the inter-professional team
- Maintain current assessments, plans, and clinical documentation
- Communicate resident care changes as required
- Schedule and participate in resident care conferences
- Evaluate care plan effectiveness and adjust accordingly
Requirements:
- Practical Nurse Diploma
- Current registration in the applicable province
- Membership in good standing
- Three or more years supporting seniors' health
- Leadership, coaching, and mentorship skills
