Provide palliative nursing care in Oakville while applying symptom management and IV therapy skills during community visits. This is a regular part-time position.
Acclaim Health seeks a Community Visiting Nurse specializing in Palliative Care. The Registered Nurse provides patient-centered care in homes throughout Halton, focusing on comfort, pain relief, and emotional support. Work includes two shifts weekly, one-in-three weekends, on-call rotation, and statutory holiday rotation.
Key Responsibilities:
- Provide comprehensive palliative nursing care in Halton homes
- Assess client health status and organize daily priorities
- Develop and modify individualized plans of care
- Administer IV therapy and prescribed medications
- Perform wound care and synchronize client records
Requirements:
- Current registration with the CNO
- One to two years community nursing experience preferred
- Palliative, hospice, or home health experience preferred
- Current CPR certificate and valid driver's license
- Access to a vehicle and strong communication skills
