Acclaim Health seeks an established part-time Registered Practical Nurse for its Oakville Community visiting team. Provide nursing care in clients' homes, clinics, and the community while supporting independence, safety, and well-being. The work includes health assessments, palliative care, chronic disease management, and collaboration with an interdisciplinary team.
Key Responsibilities:
- Provide nursing care within community and clinic settings
- Assess client health status and organize daily priorities
- Develop, evaluate, and modify plans of care
- Administer IV therapy, medications, and injections
- Provide wound and catheter care and maintenance
Requirements:
- Current registration with the CNO
- Current CPR certificate and medication administration ability
- One to two years' community clinical practice preferred
- Valid driver's license and access to a vehicle
- Excellent interpersonal, communication, and teaching skills
