Part Time Licensed Practical Nurse

Provide person-centered nursing care as a part time Licensed Practical Nurse with Extendicare, using clinical judgment and care coordination skills.

Reporting to the Director of Care or Director of Health & Wellness, the LPN delivers planned, patient-oriented, goal-directed care. You will assess residents, maintain documentation, administer treatments, and coordinate services according to provincial standards. The position supports long-term care residents through collaborative practice.

Key Responsibilities:

• Complete resident assessments, care plans, and clinical documentation
• Provide direct care and administer medications and treatments
• Evaluate care plans and communicate required changes
• Coordinate assignments, conferences, transfers, and resident services
• Manage significant condition changes and medical emergencies

Requirements:

• Current Licensed Practical Nurse registration in applicable province or state
• Strong communication, critical thinking, and care coordination skills
• Sound clinical knowledge and customer focus
• Current Basic Cardiac Life Support and First Aid certification
• Computer proficiency with Microsoft Office applications
